CTSQ is selling to its professional members carbon paper receipt booklets. Each booklet contains 50 individual receipts. These booklets can only be used by professional members who have maintained their status. The receipts from these booklets are widely accepted by insurance company and may help facilitate the claim process for some patients.
One booklet costs $5.00 (+tx). Shipping fees are added to your final invoice. We use Canada Post services for delivery. The shipping fees vary depending on the delivery address and quantity of booklets. Take a look at the table here under for more details. An invoice will be emailed once the order is shipped.
